I've found that when this happens to me, it's *usually* either when I'm testing out a new style.
Nighttime street game is one of my favorites these days, but it's a really bizarre one to get used to because it happens so much faster than almost anything else. The first 5 or 6 street pulls I had at night more or less had moments like this. You look back and you're like "Gee, that was silly."
Also happens occasionally if your mind is just elsewhere (e.g., maybe you're thinking, "Man, I've really got to meet up with Hector & co.! Everyone's waiting on me!"), and you don't fully register the signs until 15 minutes later when it's already too late... or if you just totally aren't expecting to pick a girl up and suddenly it's thrown in your lap and you're all butterfingers.
I don't know there's necessarily a good solution for the latter problems outside of just making pick up your #1 priority that takes precedence over all else no matter what so your brain is always on and ready to strike, but I don't think that's worth doing when you're at a high level. Does happen somewhat naturally to a certain extent when you go on runs, though.
