I've tried to use dental dams like the flavored ones. They suck. You can't feel anything with them.

I never use a barrier or protection with oral. I've never had any problems (not saying I won't have any knock on wood). Getting an STI from oral is relatively rare.

I personally always go down on the girl every time for about 5 minutes. That seems to be the sweet spot for timing. If she wants me to keep going I will.
